COOP REQUIRED FORMS:
- Work Education Agreement – signed by all parties and handed in
- Weekly Attendance Logs – handed in every Monday of the following week. Each credit requires 110 hours of documented placement work.
- Employer/Placement Evaluations – minimum of three (3) for each 110 hour coop credit
