In the NEXUS coop program you must complete 5 journals for each coop credit.
You can use the sentence starters at the top of each journal page to help you get started, or if something else is more suitable to your week, then write about that. You may also include items such as samples of your work, samples of notes you have received, drawings, related newspaper articles, photographs, ticket stubs from places you went or extra conferences you attended, etc., with your journal entry.

Co-op Hours Weekly Report
Guidelines
1. When a Student successfully completes 110hrs and completes the in-class assignments they are eligible for 1 credit.
2. Students must maintain an accurate record of the days they are present at a co-op placement, and of the hours worked each day. This must be done using the co-op hours sheets provided by the instructor, and hours must be verified by the student’s supervisor/employer.
3. Students must notify both the co-op employer and the school if they will be absent from their co-op placement.
4. Employers should provide a variety of learning opportunities for co-op students and provide constructive feedback to them. Student evaluation forms will be provided, and three student evaluations should be completed during the co-op placement.
5. Students should work a minimum of 12 hours per week at their co-op placement.
6. In case of injury the school and co-op teacher must be notified immediately.
